NOTICE To: All Interested Agencies, Groups, and Individuals This is to give notice that the WILLIAMSON HEALTH & WELLNESS CENTER, INC. has applied for a Health Infrastructure Investment Program (HIIP) grant from Health Resources and Services Administration (HRSA) of the Department of Health and Human Services (HHS). WILLIAMSON HEALTH & WELLNESS CENTER, INC. proposes to renovate an existing two-story building to expand their existing facility at 180 East 2nd Avenue, Williamson, WV 25661. The purpose of this notice is to inform the public that per Executive Order 11988 for Floodplain Management, WILLIAMSON HEALTH & WELLNESS CENTER, INC. recognizes that the proposed action will occur within the floodplain and that WILLIAMSON HEALTH & WELLNESS CENTER, INC. has determined that this is the only practicable alternative for this project. The proposed expansion site is adjacent to the existing clinic site, and to move to a different location is not practicable. The proposed site is on the second floor of an existing building and no ground disturbance is proposed. Therefore, no impacts to the floodplain are anticipated and no additional mitigation measures are required.
