REQUEST FOR PROPOSAL The The Cabell-Huntington Health Department is requesting the installation of an exterior sign (weatherproof) to include electric, agency logo, name, address, phone number, website, permits, materials and labor for a turn-key installation. All city, state and federal guidelines must be followed specifically the ordinances of the city of Huntington, WV. Sealed proposals will be accepted at the Cabell-Huntington Health Department, 703 7th Avenue, Huntington, WV 25701 until October 24th at 8:30 a.m. at which time the sealed proposals received will be opened and read aloud. All submitted bids must have the Cabell-Huntington Health Department RFP document signed and submitted with the proposal.
