Chambers, Chair of the West Virginia Health Care Authority, today announced that the Authority has received a Letter of Intent from Lifetree Medical Services, LLC, Cabin Creek, Kanawha, CON File #17-1/2/3/4/6/7/8/ 10-11091-P, for the development of ambulatory health care services through the provision of mid-level practitioner services at nursing home located in the following counties: Boone, Braxton, Cabell, Clay, Fayette, Greenbrier, Hardy, Harrison, Lincoln, Marshall, McDowell, Mercer, Summers, Wayne, Webster and Wyoming. The capital expenditure associated with the project is $10,000.
